  This form will need to be filled out and handed in on registration day if your student will need to take nonprescription medication during the school year.
• Prescribed Medication Form
  This form needs to be filled out and handed in at registration if your student will need to take prescribed medication during the school year.

  This form will need to be filled out and signed off by a physician and handed into the school within 30 days of enrollment.

  School supply list of items that students may need.
• 2025-2026 District Calendar
• Whom shall I contact
  This form is set up to give parents information on whom they should contact for certain areas of concern.
• Pre-Plan Absent Form
  This form should be filled out anytime you or your student knows in advance he/she will not be attending school.
• Dietary Needs
  If your student has any dietery needs that the schools and kitchen need to be aware of, please fill out this form and get it back to th nurses so they can process the information.
• Student Vehicle Registration
  Any Pardeeville student who drives to school and parks in the student parking lot will need to register your vehicle(s). You will be issued a Numbered Parking Sticker for each vehicle you register. Once registered you can pick your parking sticker up in the high school office. The Parking Sticker is good for as long as you have the car. This Parking Sticker is a Vinyl Cling and should be placed on the inside of your Windshield (front window) in the lower right hand corner (passenger side of windshield).
• Dance Guest Form
  This form should be filled out anytime a student would like to bring a guest to any of our dances. It needs to be approved by the office at least 3 days before the dance.
• Emancipation Request for 18 Year Old Students
  This form would begin the process to be emancipated for educational and behavioral purposes.
• Work Permit Link
  A work permit is needed for anyone under the age of 16 years old. Parents need to go to the Department of Workforce Development to obtain a work permit. You will need to create an ID as well as have the ability to pay for the $10 permit fee. Please click on the header to get directed to the website.
• Driver's Ed
  Driver's Ed is not available through the school. There are multiple options to choose from in Columbia County. You can find the list at: https://wisconsindot.gov/Pages/dmv/teen-driver/teen-hw-aply/schools.aspx
